PROSPECTIVE STUDENT-ATHLETE QUESTIONAIRRE

The University of Texas at Dallas established its women's intercollegiate volleyball program
in 2004 as a member of the NCAA Division III American Southwest Conference. After two
successful seasons, we are continuing the process of building an elite program and are always
looking for student-athletes who are serious about success...on the court and in the classroom.
If you are interested in playing highly-competitive intercollegiate athletics at a well-respected
public university with first-class facilities, please let us know about you by filling out the
following questionnaire...

IMPORTANT NOTICE TO POTENTIAL COLLEGE TRANSFERS:
Please be aware that NCAA Regulation 13.1.1.2 allows us to contact a four-year college prospect ONLY AFTER WRITTEN PERMISSION has been obtained from the institution from which the prospect is transferring. Additionally, Regulation 13.1.1.2.1 requires that a prospective transfer from another DIII institution must present a SELF RELEASE before we can talk with them. This means that - even if you send us this form - we cannot contact you until one of these other processes has been completed.
